Patient and client intake

AI patient and client intake for clinics and firms

Intake forms, emails and scanned documents are read, checked for gaps and entered into your practice system before the first appointment.

01 today

The problem

Front-desk staff retype details from online forms, emailed scans and paper sheets into the practice system. Missing referrals, ID or signed consent come to light on the day, and the appointment starts late or gets moved. The same questions get asked twice.

02 the run

What the AI does

  1. Step 1: Picks up new intake forms from Jotform or Typeform and documents sent to the shared inbox.
  2. Step 2: Reads names, contact details, history and reason for visit from forms and scanned files.
  3. Step 3: Checks the file against your checklist for that service, such as referral letter, ID or signed consent.
  4. Step 4: Emails the client a friendly request for anything missing, with a secure upload link.
  5. Step 5: Creates or updates the record in Cliniko, Clio or Karbon and attaches the documents.

Where a person approves

Front-desk staff confirm each new record before it is created and review any field the system was unsure about. Clinical or legal judgement on the intake stays with the practitioner.

04 after

What changes

Files are complete before the client walks in. Reception spends less time retyping and more time with the people in front of them.
